Out-of-State Patients

General Information

  • The application process is completed online

  • OSP’s may apply for no more than two, 60-day terms in a calendar year

  • May request a “start date” for the term to begin

  • May apply 60 days prior to the requested “start date”

  • Application fee is $49.50 and is non-refundable

  • Electronic access to the Hawaii 329-OSP Registration card will be provided upon approval

OSP Requirements

  • Valid medical cannabis registration card from another U.S. state/territory or District of Columbia

    • If your medical cannabis card does not have the expiration date on it (i.e. New York), please upload additional documents that state the expiration date of your registration in the “other documents” section in the Hawaii online application.

  • Valid government-issued ID or driver’s license from the same US. state/territory or District of Columbia* with the same name on the medical cannabis registration card.

  • Certified in your “home” state as having one or more debilitating medical conditions identified in Hawaii State Law

*Passports, military ID cards, or any other ID are not accepted and the application will be denied if submitted. 

Here is the step-by-step application process for an OSP:

1)    Go to www.medmj.ehawaii.gov

2)    Create an Account

3)    Select “Out of State Application”

4)    Fill out the Online Application with:

a.     Patient Information

b.     Caregiver Information (if applicable)

c.     Certifying Medical Provider Information

5)    Upload supporting documents for each

6)    Pay the Non-Refundable Fee of $49.50

7)    Review and Submit the Application

8)    Once the Hawaii State Department of Health approves the application, they will send an email where the patient may download their Medical Cannabis OSP Registration Card.

9)    The patient must have their OSP Registration card and State ID available upon entering a Hawaii State Medical Cannabis Dispensary.

OSP Minor ID types

  • Birth Certificate with a unique ID number usually found on the top-right corner**.

  • Valid state/territory ID

  • Valid driver’s license

  • Valid passport book (not passport card)

OSP Minor’s Caregiver – Additional Requirements

  • Must be the parent, guardian, or legal custodian of the minor applicant

  • Valid government-issued ID or driver’s license from the same US. state/territory or District of Columbia as the minor’s valid medical cannabis registration card***

***Passports, military ID cards, or any other ID are not accepted as valid government issued ID’s for caregivers and the application will be denied if submitted.

We do not accept international medical cannabis registration cards at this time.

Qualifying Conditions

As defined in section 121 of HRS-329, a qualifying debilitating medical condition in the state of Hawai’i includes:

  1. Amyotrophic Lateral Sclerosis

  2. Cancer

  3. Glaucoma

  4. Lupus

  5. Epilepsy

  6. Multiple Sclerosis

  7. Rheumatoid Arthritis

  8. Positive status for human immunodeficiency virus

  9. Acquired immune deficiency syndrome

  10. Post-traumatic stress disorder, or

  11. “The treatment of these conditions”, or

  12. “A chronic or debilitating disease or medical condition or its treatment that produces one or more of the following:

  • Cachexia or wasting syndrome,

  • Severe pain,

  • Severe nausea,

  • Seizures, including those characteristic of epilepsy, or

  • Severe and persistent muscle spasms, including those characteristic of multiple sclerosis or Crohn’s disease.
